IMPORTANT – PLEASE READ CAREFULLY BEFORE PARTICIPATING IN ANY SESSION

By booking or participating in a 15-Minute Health Coaching session (“Session”) with Dr. Hanz, you acknowledge, understand, and agree to the following terms:

1. No Physician–Patient Relationship

I understand that participation in a 15-Minute Health Coaching session does not create a physician–patient relationship between me and Dr. Hanz. The information provided is for general educational and informational purposes only and is not intended to diagnose, treat, cure, or prevent any disease or medical condition. I understand this session is not a substitute for professional medical care from my licensed physician or healthcare provider.

2. Personal Responsibility and Medical Decisions

I agree to consult my own licensed healthcare provider before making any medical decisions or changes to my health, medications, diet, or exercise based on information discussed during the session. I accept full responsibility for how I interpret and apply any information shared.

3. Limitation of Liability

I acknowledge that Dr. Hanz and any affiliated representatives are not liable for any injury, loss, or damage (including but not limited to physical, emotional, or financial harm) that may result from my participation in the session. I release and hold harmless Dr. Hanz from any claims, actions, or liabilities arising from participation or reliance on the session’s content.

4. Recording and Use of Session Content

I understand and agree that the session may be audio and/or video recorded by Dr. Hanz or his representatives. I grant full rights to Dr. Hanz to use, reproduce, distribute, and publish these recordings (in part or in full) for educational, promotional, or commercial purposes, including but not limited to social media, online courses, podcasts, and marketing materials. I acknowledge that: I will not receive compensation for such use. I waive any rights to review or approve how my image or voice is used. If I do not wish to be recorded, I must notify Dr. Hanz in writing prior to the session, understanding that this may make me ineligible to participate.

5. Nature of the Coaching Service

I understand that 15-Minute Health Coaching sessions are educational and motivational and do not include diagnosis, prescription, or treatment of any kind.

6. Consent and Acknowledgment

By agreeing, I affirm that:

I have read and fully understand this agreement.
I voluntarily consent to the recording and possible use of the session.
I voluntarily participate and agree to all terms herein.
I am at least 18 years of age (or have legal guardian consent).
